- Description
- World Trade Center site identification card issued to Fredrick Lewis of VMAT-2. The card includes a photograph, the logos for the National Disaster Medical System and the U.S. Public Health Service, and printed text: "FREDERICK LEWIS | VMAT-2 | Department of Health and Human Services | U.S. Public Health Service | National Disaster Medical System | WTC | New York | 09-11-2001." The card includes an image of an American flag and the Twin Towers.

- Following the events of September 11, 2001, Dr. Frederick Lewis, Jr. was deployed to Ground Zero as the acting Team Commander of the Veterinary Medical Assistance Team 2 (VMAT-2). He and his team provided medical attention to search-and-rescue dogs, police horses, and other injured animals found near the site.
